On Win9x you do have direct access to the hardware, and I once compiled one of my QB45 tools in FreeBASIC and while things ran, they were choppy and fair bit slower... DOS program performed quite a bit more nicely (andi n Win9x, DOS program gets majority of CPU time so it will run quite smoothly, but max performance and stability is still coming in pure DOS).

You can get full access to ports under win 2K/XP using the UserPort driver. It modifies the port access table so that specified port ranges can be written to. I have used this to allow an OPL2 tracker to access my sound blaster 16 from win2k. Any type of program will have full access to those ports dos/win16/win32.
